Hospital Referrals

On some occasions it may be necessary for your GP to refer you to see a specialist. If this is the case your GP will discuss all of the different options available to you. Once you have decided where you would like to attend or the different options that you would like to choose between, the GP will ask the practice secretaries to make the referral. The practice secretaries can be contacted by telephoning the surgery.

The practice uses the NHS Electronic Referrals System (ERS) to refer our patients. The ERS is a service that allows you to choose your hospital or clinic and book an appointment with a specialist when your GP refers you. Our practice secretary will send you all of the information that you will need to use the ERS.

Hospital Passport

A hospital passport is a personalized document that provides hospital staff with important information about an individual's health needs, communication preferences, and personal details, such as likes, dislikes, and interests. It helps ensure the patient receives appropriate and comfortable care by informing staff about how to support them, make reasonable adjustments, and manage their conditions effectively.

Disability Distress Assessment Tool (DisDAT)

A DisDAT tool, or Disability Distress Assessment Tool, is a method for identifying and documenting the signs of distress in individuals who have severely limited communication skills. It helps caregivers and professionals recognize subtle changes by first establishing a baseline of what the person looks like when they are content, and then documenting their specific cues when they are distressed.
