If you have been ill for 7 days or fewer
If you have been ill for 7 days or fewer you do not need to consult with a doctor. You should complete the self certification form below:

If you have been ill for more than seven days
Please contact us online below to complete an online admin form Fit (Sick Note). Our team will then review your request within two working days and send you a fit note if appropriate.
If you are unable to complete the form online you can call our team on 01509 324115 after 9am and they will be able to submit a form on your behalf.
